This changelog hasn't been updated for a long time. Currently the change notes are stored in the GitHub releases: https://github.com/timescale/timescaledb-toolkit/releases This removes the old changelog and adds a link in the README indicating where the changelog is stored now.

This changelog hasn't been updated for a while (since 0.2.0). Currently the change notes are stored in the GitHub releases, which has the same information for the two releases that are in the changelog, and has information for all releases past 0.2.0. As far as I can tell the old changelog isn't linked to from anywhere.
This PR removes the old changelog and adds a link in the README indicating where the changelog is stored now.
