It now appends all new content after the currently executing script element, if called during the first tick of that script (that is, before control is returned to JSDOM). This replaces checking whether the page is loading or not.
This should fix #500 well enough.
It breaks the Sizzle test with an uncaught exception that `this.CSSStyleDeclaration` is `undefined`. But only when running the tests together, not when running the sizzle tests alone! A very bad sign.
