Somehow - I am not sure how - I noticed that jobrunhistory.db got corrupted where the friendly name was missing from the line; resulting in a parsing error and general problems. I think it fell into a #2-like behavior, as the thing that caused me to investigate was that I was receiving a report that it could not send mail. (It could.)

The solution here is probably to catch and handle errors during parsing more intelligently.
