    problems when e.g. generating content to be included in an Atom feed.
    In contrast, " " is valid in all contexts.

    Before this I'd get a failure like the following when Python 2.7 running called out to '/usr/bin/python2.5 ...'. Looks like
    Python 2.7 sets MACOSX_DEPLOYMENT_TARGET in its env, which is
    inherited for "os.system" calls.

    so that TOC can be generated properly from a mixed-markup document.
    With the current code the TOC for the following document:
    is generated as: hb ha hc, while the correct order would be ha hb hc
