Escaping underscores producing md5 hash #15

When writing the following:

## GET /posts/:post\_id/comments/:map\_id

It's producing an md5 hash instead of an underscore:

GET /posts/:postmd5-e60453642b5f53da16f4e11348bc05c1id/comments/:mapmd5-e60453642b5f53da16f4e11348bc05c1id

We need to escape /posts/:post_id/comments/:map_id otherwise it places emphasis tags around the middle of the URL.

python-markdown2 2.0.1 fixes the underlying issue (trentm/python-markdown2@146e858)

@trentm [issue #15] Unescape escaped chars in Table on Contents entries.
The underlying issue was (fixed in markdown2 2.0.1).

Fixed in restdown 1.2.20. Thanks! and sorry for the wait.

Found other issues with headers using Markdown escapes. Take a look at restdown 1.2.21 instead.
