mrmartineau css/scss: better 1st & 3rd party module support, other amends
## Improved
- Sass dependencies can now be installed from npm, this means we can update and maintain them independently from the main kickoff repo and even `@import` 3rd party sass libs with ease.
  - imported libs
    - https://github.com/TryKickoff/kickoff-grid.css - Kickoff’s grid
    - https://github.com/trykickoff/kickoff-utils.scss - mixins & functions (*breaking:* all mixins & function names are now use the `ko-` namespace prefix, e.g. `ko-clearfix`)
    - https://github.com/TryKickoff/kickoff-fluidVideo.css - fluid video component
    - http://include-media.com - improved media-queries
    - https://github.com/JohnAlbin/normalize-scss - sass port of normalize.css
- Code comments are now more consistent.

## Removed:
- grid: it has been extracted into its own npm module, see https://github.com/TryKickoff/kickoff-grid.css
- mixins & functions: they have been extracted into their own npm module, see https://github.com/trykickoff/kickoff-utils.scss
- fluid video component: it has been extracted into its own npm module, see https://github.com/TryKickoff/kickoff-fluidVideo.css
- media object: our flexbox grid is better at handling this now, see https://codepen.io/mrmartineau/pen/NRVaoB for an example
- _dependencies.scss - as we no longer need multiple css builds, we can safely bring the contents of this file into `kickoff.scss` and remove it
- some image modifier classes
