  2. Update calendars data (CLDR v21).

    Changes:
     - added missing abbreviated months names (e.g., in de locale)
     - updated day periods (am/pm) handling (basic implementation)
     - check that buddhist calendar exists in th locale (it's erased when
       resources are regenerated with ruby-cldr as it supports only
       gregorian calendars)
     - updated specs where CLDR data has changed (e.g., comma after weekday
       in es full date format, or full time format in ko locale)

  3. Fix date and time order in DateTimeTokenizer placeholders.

    According to http://cldr.unicode.org/translation/date-time-patterns
    (see section 'Date-Time Pattern') '{0}' is equivalent to '{time}' and
    '{1}' - to '{date}'. That means first placeholder (with 0 index)
    should be time and the second one (with index 1) - date.
