Add versioned Forms default theme files

📋 Description

Previously, updates to the default theme files were shared as Source Code on Our: https://our.umbraco.com/packages/developer-tools/umbraco-forms/. Short URLs were used in the documentation to link to the latest file for a specific major: this required adding the file to Our and updating the short URL for each update and the creation of new short URLs for each major.

I've now added the ZIP files to the documentation and listed every available version for the respective Forms major. Versions 15 and 16 didn't have a short URL yet, so this PR makes them available now.

Although this doesn't fully address the issue raised in umbraco/Umbraco.Forms.Issues#1266, it does ensure all available versions are listed per major.

FYI: I won't be creating a separate PR targeting the umbraco-eol-versions branch to apply the same update the 11 and 12 majors, as they aren't listed anymore, definitely won't get any new versions and the files can still be downloaded using the short URLs or from Our...

Good catch! That file doesn't exist, so the previous (rc1) version should be used instead. This is the same for version 10.5.0: the changes to the theme files were already part of the RC and users testing this version would need those updated files. However, the stable release didn't require any changes to the theme files, so the 10.5.0-rc1 version would then still be the latest version...
