VSM temporary files are not always deleted #2115

Under some situations (e.g. mmap failure) _.vsm.$PID files are not deleted.

They should be removed on failure.

Kill any temporary vsm files on failure
Use MGT_complain() to log any errors; we might have a better chance to
understand what's going in.

Fixes #2115.
