Deprecate XHTML 1.0 export #760

When saving, it's still possible to export to XHTML 1.0. This is not great, a) xhtml 1.0 is long deprecated, and b), it strips out a bunch of structural tags from the document.

(yes, we would keep the XHTML5 mode)
