Account for a headline element attribute for full and excerpt views

When a full item or excerpt item is displayed, it's likely accompanied by a title. There should be the opportunity to wrap that title in a header element that is semantically appropriate. If a header element level is not defined when the shortcode is used, it should probably fallback to span.
