Enforce game-level bans by username as well. #4139

This will have each game track a list of banned usernames in addition to currently tracking banned IPs.

Thanks! Now that we require registration this makes a lot of sense.

Perhaps we should commit this to 1.14 as well.

I think that would make sense. While it's not frequent, it is something I've seen happen occasionally on the 1.14 server and originally why I added "If the host of a game bans you, don’t attempt to evade the ban" to the MP CoC.
