Create an issue template

I routinely have issues posted that are about regressions that I have already fixed (#374) or about things discussed in official(s) threads and/or do not contain any info (#367). Contributing md is fine but not enough - we need an issue template (in a .github folder):

Someone get me started and will finalize it @wrye-bash/active. Add the info from https://github.com/wrye-bash/wrye-bash/blob/dev/Contributing.md ( see also https://github.com/wrye-bash/wrye-bash/wiki/[github]-Reporting-a-bug ) - request a link to the threads (linked in questions/feedback so we edit the links to threads only there), a bugdump and rest of info
