Add support for the "nopanel" parameter in excerpt-include macro #195

The nopanel parameter is used in Confluence for extra decoration. If it's value is true, then there are some borders and a background color on the heading. When the parameter's value is false, besides the missing borders, the heading containing the title is also missing, making the excerpt easy to disguise in content.
