Make sure you are using the latest version: run youtube-dl --version and ensure your version is 2016.10.07. If it's not read this FAQ entry and update. Issues with outdated version will be rejected.

Description of your issue, suggested solution and other information

Sometimes the title of the video page will have encoded HTML entities, such as http://www.lego.com/en-us/starwars/videos/lego-star-wars-force-surprise-9685ee9d12e84ff38e84b4e3d0db533d or http://www.lego.com/en-us/city/videos/lego-city-movie-mixer-mash-up-volcanic-vandals-b7e5ec40b18949a2be30604953fbb504, therefore the title of the video download will have similar encoding.
