[ccc] media.ccc.de support is broken, changed URI format

media.ccc.de support is broken.

As it seems, the URL format has changed. Also ccc.py does not use the API which is documented in the README of the project.

Here's the log although it is quite uninteresting as is just defaults to the generic extractor.
