CDN での分散を想定しています。
遅延は、3秒程度に抑えることができます。
キーフレームは、 0.5~1.0 秒程度に設定される必要があります。

CDN で効果的にキャッシュしてもらうにはコツは必要か。

少なくとも、 nginx-rtmp-module で運用するのは現実的ではない。
