-r (or -R or --recursive) flag in pdfgrep is not working, it does not grep recursively down paths

Did you check upstream if it could be a bug in the version you're using? Generally when there is an issue in the way brew packaged software it results in crashes. So I'm not sure brew can help here.

Also, how do you know pdfgrep is not recursing down your directories? If it's because "I'm not seeing any output", are you sure your PDFs are searchable in the first place? Scans of text documents generally aren't searchable, unless whoever/whatever created them also did OCR on the scans before saving the final PDF.
