vscode vsce does not support pnpm

Need to figure out a way to package the extension as a single bundled .js file. no depdendencies.

• upgrade vsce
• set the --no-dependencies flag to avoid the built-in bundler in vsce
• bundle all depedencies into the compiled vsix package
