Tool failed

The tool gets picked up in agent mode now. Got this. It's true that I didn't have microsoft/vscode open. I was in a workspace with no git repo open. Maybe this tool should have a when clause to be disabled when it can't be used?

I don't even know why it picked microsoft/vscode, my name must be in its training data...

I tried to add this, but it seems like the when for a tool is only evaluated once. If the result of the when changes from false to true should the tool get added to vscode.lm.tools?

It should be evaluated every time it changes, then show up in the list. We use this for some builtin tools so I'm expecting it to work. But maybe there's some bug.

Sometimes it seems to be re-evaluated, and sometimes not:

• It is re-evaluated when the when is only based on settings
• It isn't re-evaluated when the when is based on other, extension provided, context keys.
