Blank page after creating issue

when creating a issue on VSCode's Github Repo
I'm left with a blank page
doesn't happen on other repo's
only VSCode

1. Create a issue on VSCode's Github Repo
2. click Create
   
3. Blank page

same issue with https://github.com/eemeli/yaml

however https://github.com/redhat-developer/vscode-yaml is fine
and so was https://github.com/frizLabz-FFriZz/Pine-Script-v5-VS-Code
