Changes in mcp.json are not detect until reload

1. Have an MCP sever in your mcp.json
2. Comment it out
3. 🐛 the MCP server still exists until you reload the window

Actually it seems like changes in general in mcp.json are not picked up without a reload

Tried to look into it, I'm not sure where this is going wrong, I can't seem to find the code that should be reacting to changes -- is it just not implemented?

Yes. This is not implemented. I underestimated how much users rely on editing the json file. Implemented and fixed it.
