Why:

Fix #660 - incorrect links to CONTRIBUTING.md and CODE_OF_CONDUCT.md

What's being changed:

Update the links to CONTRIBUTING.md and CODE_OF_CONDUCT.md in issue template so that they don't lead to "404 page not found".

Check off the following:
