VSCode Swift docs still refer to Code-LLDB

To Reproduce

2. Go to https://code.visualstudio.com/docs/languages/swift
3. See Code-LLDB references

Expected behavior
We should reference lldb-dap instead

It would be nice if we ported over the lldb-dap section from this article to the emacs and vim pages...
