Files and directories created with external tools don't appear in File Explorer

Steps to Reproduce

1. Create an MCP server with a tool that creates a NEW directory or a file within the current workspace and configure it to work in the current workspace.
2. Trigger the tool call from Agent chat, so that the directory/file is created on disk.

Expected Result

User should immediately see the new file/directory appear in the File Explorer, as soon as it's created.

Actual Result

File / directory do not appear in explorer until the file is explicitly navigated to (e.g. if the LLM responds in the chat with a hyperlink to what directory/file was created, and the user clicks the link, then they will be correctly brought to the file and the file will then appear in the explorer).

EDIT: This seems to be a problem with any external tool (e.g. creating files with integrated terminal, MCP server, etc), so I've updated the title.

Are you asking that we reveal it in the explorer? As the file is there, just we don't scroll the explorer. Or are you seeing different?
