Issue Reporter - Add more information to the submission form (i.e. submitting Username and target repository)

• On the submission field, the username that is going to report the issue on GitHub needs to be shown.
• Maybe the ability to change the username that is going to make the bug report. (I am not 100% about this one)
• Show the repository that these issues are going to be posted to. (i.e. microsoft/vscode)

Reasons

• As new user I would like to see that is it my GitHub username that is going to be used so I can check or edit the issue at a later date.
• As a new user I would like to know where these issues are going to be submitted.
• Developers might have different uses for their usernames.
