Add an environment value to represent Crowi URL

Overview

Until v1.7.9, Crowi's URL was calculated from the actual request. It will be a cause of making updating configs logics more complex on multiple apps environment. #475

In this ticket, we change the URL that referred by apps to the environment variable instead of the initial request.
