User registration for the DiADeM service
This privacy notice is directed towards persons who sign-up with the Apperta Foundation CIC (https://apperta.org/) to be a Registered Users of the DiADeM service. The privacy notice is NOT applicable to patients who disclose personal data as part of a DiADeM assessment.
DiADeM (Diagnosing Advanced Dementia Mandate) is a dementia diagnostic tool to support the diagnosis of people living with advanced dementia in care home settings. It is designed for use on people living with advanced dementia within a care home who do not have a formal diagnosis. The tool was developed in 2015 by the Yorkshire & Humber Dementia and Older Peoples Mental Health Clinical Network (YH DOPMH CN).
In signing up to be a Registered User of the DiADeM service, Apperta requires that the Registered User supplies personal information.
Apperta will retain this information whilst you remain a Registered User of the DiADeM service. You are able to view online all information that we retain about yourself using the tools on the DiADeM User portal. Apperta will retain all your information on secure cloud storage systems within the EEA region.
Apperta may make available certain user information for the purposes of research and audit to third parties. The information made available will not include contact information without prior permission being provided by yourself.
If at anytime you cease to be a Registered User of the DiADeM service, we will remove all your personal information held by Apperta concerning your registration for the DiADeM service.
Apperta Foundation CIC (www.apperta.org) is the Data Controller of all personal data held regarding your registration for the DiADeM service. If you have any concerns or questions regarding the retention of your personal data, you may contact Apperta at info@apperta.org.
