Pain blend vision shows as obstructive solid rectangles when r_lightmap is enabled. #93

This occurs on the "opengl2" renderer only, on the "openggl1" renderer the pain blend shows as intended.

That happens when you set r_picmip to a high value iirc. I think it happens on openGL1 renderers too (I can test this properly l8r, im away from home atm)

I just tested different values of r_picmip with lightmap enabled on both renderers. On opegngl2 the bug still occurs even with r_picmip set to 0. On opengl1 the bug doesn't occur with settings from 0 through 5 for r_picmip.
