Strictly speaking Formhub is no longer maintained. We are currently working on the next generation of Formhub at https://github.com/SEL-Columbia/dokomoforms

If you still want to use Formhub, you can deploy on your own server. Following the wiki page:

https://github.com/SEL-Columbia/formhub/wiki/Installation-and-Deployment

You can install Formhub manually by following this guide:

https://github.com/SEL-Columbia/formhub/blob/master/docs/install/README.md

... or using a virtual machine:

https://github.com/SEL-Columbia/formhub/wiki/How-To-Run-Your-Own-Formhub-Virtual-Machines-on-VirtualBox

...or you can install Formhub on Amazon Web Services:

To add to this:

Formhub was forked by KoBoToolbox and Ona (by most of the original formhub
developers). From what I can tell, these projects are booming and
tremendously awesome. They both continue to use the same solid and popular
form format. I recommend considering those instead of formhub.
