These can greatly increase the number of items without contributing to TMJ or messing up item distribution.

For example, you can have the prefix "rusty" for armour, which might have extra to-hit penalties, or "superior" armour which has a lower weight.

Similarly, elven swords, orcish picks, or dwavern war hammers.

These are all done (or at least underway) in http://github.com/magnate/angband/tree/affixes.
