I have reported this to DO for help.

tl;dr one or more times a week our database spikes from ~3 GB to almost ~30 GB for 45 minutes to an hour. I have grepped logs and I don't see any traffic spikes or obvious errors.

This is what DO sees:

This was more random pre-switching to django-q2 so I don't believe that it's related. Plus, we aren't sending many jobs to it.
