Two contracts deployed instead of one #3181

Version: `0.9.2`
OS & Version: linux
Node version: `geth 0.7.2` 
Number of blocks synchronized: 4,382,439

Today I deployed contract using MIST 0.9.2 I entered password and clicked deploy button only once but I got TWO duplicate contracts in main network!

This is totally weird bug, we need to figure out what to do now.
