Strange, seemingly random error: There is no request token for this page

One of my users reported seeing this when trying to sign up with Twitter:

Whoa there!

There is no request token for this page. That's the special key we need from applications asking to use your Twitter account. Please go back to the site or application that sent you here and try again; it was probably just a mistake.

I have never seen that bug before and most users seem to sign up just fine. I was wondering if this might be a possible bug with passport-twitter or something else.
