for each group of DOI on the detailed form, have a checkbox for people to tick, which unhides the entry boxes.

original spec: - make tickboxes for each group of DOI eg - pharma yes/no , devices yes/no, patents yes/no, CCG yes/ no , private practice yes/no, and then a box to enter underneath

The current declaration format should become read only historical info.

Create a new declaration format along the lines of:

Drop down menu offered for categories of declarations with blank spaces for text

Clinical work
-academic positions + employer
-NHS positions +employer
-Private clinical work +location/employer

Consultancy
-pharmaceutical companies
-technology companies
-other

Academic relationships
-research grants
-other

Other work
-Public relations
-commercial relationships
-media

Financial
-patents-owned/part owned
-shares/stocks/company ownership

Spousal/family
-freespace to enter

Sponsored/educational events
-sponsored education
-conferences
-meals/hospitality/travel

Political/membership organisations of note
-free text
