Tested with phpmd 1.5.0,

the generated html report links to the following non-existing documentation:

http://phpmd.org/rules/design.html#staticaccess

(the #staticaccess anchor is missing)

@martinlindhe the whole clean code section is missing.
My PR #201 should resovle this.
