support sub directories

Please add pagination support in sub directories. E.g. /blog/index.html -> /blog/pageX

that's currently out of scope for this plugin, sorry. it's a bit tedious to extend Jekyll in a clean and orderly fashion, in particular when you want to keep your modifications configurable. so we have to reject this feature request for now.

we might come back to it in the future, though. patches/pull requests welcome.
