Fixed handling of the "first chapter line" to avoid dropcaps in the second paragraph of the chapter (when first paragraph starts with formatting styles)

Dropcaps and notes formatting

Make sure that inline notes format is properly defined by css (and not by python)
Fix pagination - in some cases page calculation was preventing proper dropcap processing
