Agent attempts to save untitled file after making any changes to it

Steps to Reproduce:

1. Cmd/Ctrl + N to create a new untitled doc.
2. Ask Copilot Agent to generate anything (like fibonacci function).
3. After content insertion in the file, VS Code prompts to save the file via standard save dialog.

Quokka.js extension uses scratch files (untitled&unsaved files) for some of its features, and the save dialog on every agent change makes it very inconvenient to use agent with those files.
