Make accessibility fixes

Running list of what needs fixing:

• Add :focus styles for elements in header
• Panel: After tabbing out of the panel, it should go to the next row, instead of the bottom of the table
• Add :focus styles to form elements
• Ensure placeholder content is high enough contrast
• Make accordion buttons have spans that change text on expansion
• Add 'aria-expanded` to accordion
