Describe the bug
The voice log shows a blank embed as soon as the user starts/ends the stream in a voice channel.
It does not show that the user has ended the stream while streaming and directly disconnecting the voice channel. (see Log screenshot)

To Reproduce
Steps to reproduce the behavior:

  1. Set d!log voice enable to a log channel
  2. Connect to a voice channel.
  3. Start the stream in the voice channel.
  4. End the stream in the voice channel.
  5. Start the stream in the voice channel.
  6. Disconnect from the voice channel.

Expected behavior
It should show that the user has started or ended a stream in a voice channel and while streaming and directly disconnecting it should show that he has been streaming.
