For the top banner, I propose freedownloadmanager.org##.ab1. The ad square is already supposed to have been fixed by a dozen entries in AdGuard Base and EasyList.

Also, if the OP sees this, they should turn off https://easylist-downloads.adblockplus.org/abp-filters-anti-cv.txt, after that list began adding a whole lot of AAB whitelistings some weeks ago.
