Looks like the fields aren't large enough for the default values for usage and storage type:

Definitely caused confusion for folks looking at this for the first time.

Just checked again in a couple different browsers, and they are still cut off...

@tlwhit2 please provide screenshots of each of the browsers you tested. Include the browser name and the OS. This will aid us in adjusting the field widths.
