Update "Relationship to Facility" to drop down list with one free text field

User Story

As a user of the HealthCare RollCall UI
I'd like to have a list of options, in addition to a free text field when I define a contact's relationship to a facility
So that I can have options when assigning relationships and will have consistent relationships throughout the system

Acceptance Criteria

• Link to Relationship on Contact Information Page is a drop down list with the following options - Owner, Manager, Delegating Nurse, Property Manager, Regional Manager, Service Coordinator, Site Manager, Tenant Council President, Assistant Property Manager, Director, Other (allow text entry)
• Link to Relationship Option "Other" allows text entry
• Each of the Relationships listed above in AC 1 can be assigned to a Contact-Entity Relationship and is visible on the Facility Check-In page
• The free text "Other" option in AC 2 can be assigned to a Contact-Entity Relationship and is visible on the Facility Check-In page
• Relationships can be updated from one type to another
• PR merged to master
