Name of the city doesn’t display in the applicant adress on the last page of the opinion doc.

Subject of the issue:

On the last page of the document 'imię_nazwisko_INDYWIDUALNE', adress of the applicant doesn't include name of the city. It doesn't matter of it's long/short, it just doesn't display.

Steps to reproduce:

1. Fill in the form with valid data.
2. Click on the 'Utwórz dokument'
3. Go to the last page of generated document 'imię_nazwisko_INDYWIDUALNE'.

Expected behaviour:

Adress of the applicant should be complete.

Actual behaviour:

Adress of the applicant includes proper data that was filled in the form, except there's no city.
