Name/names of the applicants/parents doesn’t update in the protocol.

Subject of the issue:

When changing a name of the parent/applicant in the form (instead of Tomasz Rzeźniczak and Adelajda Kieł ;)), it updates at the top of the document 'imię_nazwisko-PROTOKOL', but in the middle part, it stays the same (it happens also when I delete the second parent data or change the second parent name).

Steps to reproduce:

1. Fill in the form with valid data, changing name 'Tomasz Rzeźniczak' for sth else and delete the second parent data.
2. Click on 'Utwórz dokument'.
3. Go to 'imię_nazwisko-PROTOKOL' doc.

Expected behaviour:

Applicants data in the protocol should be updated and consistent with the form.

Actual behaviour:

Applicants data are updated in the two of three parts of the protocol, where by default it shows 'Tomasz Rzeźniczak i Adelajda Kieł'.
