Link names of diseases (or diagnosis) to ICD-10 codes and patient ID number

Can a column containing the names of diseases (diagnosis) be added to a data frame containing the patient ID number, ICD code, and date of service/diagnosis? I used the "short" database, but it does not add the patient ID number. I am not looking to use comorbidity groups. Thanks!

Because the original sources of ICD-9 and 10 codes from https://hcup-us.ahrq.gov/ are broken, we are finding an alternative source of the names of the diagnosis. They will be added in the next update.
