[Bug] Navigating between activities using the nav menu doesn't update the Activity Name

Description

In HITECH and MMIS, the activity name field does not update with the correct name. However, any updates to that name field will cause the incorrect activity name to update; however, any updates to any other field (even with Autosave) does not update the activity name.

Steps to reproduce

1. Open an MMIS or HITECH APD after logging in
2. Create a new activity so there are at least 2 in MMIS or 3 in HITECH
3. Click on APD overview from the left navbar for Activity 2, provide it a name
4. Click on APD overview from the left navbar for Activity 3, notice the rest of the page updates, but the activity name doesn't
5. Make an alteration to the activity name and see activity 3's name will update to the existing (and incorrect field)

Expected results-
Navigating to the activity overview page should update the entire activity overview page with the appropriate data

Actual results-
Navigating to the activity overview page from a different activity updates everything except the activity name.
