Despite user being able to edit description, only file title (altname property) is added, file description (description property) is not added

Also "Use captions" needs to be updated to more options:
No
Yes --> Yes, Title
[NEW] Yes, Title + Description (making this new default)

Added with 3cef4d0, but without parameter
