Add persistent continuation to indexing by index & parallel indexing #1107

If a server/process halts during "by index" indexing, allow a new process to continue the indexing from a saved point instead of restarting from scratch.
By diving the work to ranges, set the infrastructure for parallel "by index" indexing.
